Partner Profile: Portico Healthnet
In 1995, HealthEast Care System created Portico Healthnet in response to the growing number of low-income east metro community members who couldn’t afford the cost of health insurance and didn’t qualify for government sponsored health programs. By providing coverage for primary and preventive services, Portico was able to improve participants’ health and address the high cost of emergency services and inpatient care to uninsured individuals.
Today, Portico helps low-income people, people of color, immigrants, and other uninsured Minnesotans enroll in health insurance and navigate the complicated health care system. Our certified MNsure Navigators provide free, personalized assistance and help clients find the most affordable coverage for their family.
Founded in 1996, the NWHFSC is a Minnesota state-initiated collaborative that pools resources and knowledge to provide health and human services. At NWHFSC, our work is to provide family advocate services, immunizations, preventive mental health, and resources for families and children. Three school districts and their associated nine cities comprise the Northwest Hennepin Family Service Collaborative area. School district representatives, Hennepin County staff, healthcare businesses and community members serve on the Board of Directors. Learn more at www.nwhfsc.org.
We welcome new partnerships. If you know of organizations able to provide services for residents in Brooklyn Park, Brooklyn Center, Maple Grove, Osseo, Champlin, Dayton and portions of Plymouth, Rogers and Corcoron, please email nwhfsc@district279.org.
